Completing the NE Abuse Form when Performing a Background Check

One Source offers a Nebraska (NE) Abuse Registry Search. Where does this information come from, one might ask. The NE Abuse search is conducted through the Department of Health and Human Services (DHHS). According to the Official Nebraska Government Website, “The Nebraska Laws 28-718 and 28-376 states the Department of Health and Human Services (DHHS) must keep records of individuals who DHHS or courts find responsible for abuse and neglect of a child or vulnerable adult. DHHS maintains these records in the Nebraska Adult and Child Abuse and Neglect Central Registry.” The search does not include any criminal conviction information and unless required by law, we do not recommend it as a primary search.

Tips one should know when completing the NE Abuse form

  • Form needs to be filled out completely.
  • Email address is strongly recommended, however is not required.
  • The applicant is required to list all other names they have used, this includes nicknames. If not applicable, have the applicant put N/A so DHHS will know it was seen by the applicant.
  • The applicant is also required to list names and birth dates of their children or children that have lived with them. The date of birth is required and often overlooked. If the applicant has not had any children residing with them at any time, then the applicant needs to fill in with N/A.
  • Please have the applicant list all past Address. If they do not have past addresses, they should put N/A for clarity.

DHHS requires the same attention to detail on the second page.

  • Make sure the appropriate boxes are checked by the applicant to state they are consenting to their names being checked in each registry.
  • The applicant then needs to sign their legal name and date. The applicant’s signature cannot be electronic.
  • Section A or B needs to be completed too. If section A is completed, then section A needs to be notarized. What if it is not applicable? If section B is completed, the client needs to sign/print their legal name and date. Again, the client’s signature cannot be electronic.

Other useful tips when completing the NE Abuse Registry Form

  • There are times the applicant you want the search done on is under age. If this is the case and the applicant is under 19, the legal guardian needs to sign and date the form on page 2. Make sure the information below section B is completed if the applicant is a minor (under the age of 19). If this box is completed, it does need to be notarized.
  • If there is anything on the form crossed out, the applicant needs to initial and date next to the information.
  • Electronic signatures are not accepted.
